Community Needs Assessment | Niagara Falls Memorial Medical Center | Niagara Falls, NY

The Niagara County Department of Health (NCDOH), in collaboration with the Niagara County Department of Mental Health and Substance Abuse, Catholic Health and Niagara Falls Memorial Medical Center, has prepared the Community Health Assessment (CHA) and Community Health Improvement Plan (CHIP)/ Community Service Plan (CSP) for the 2025-2030 period. This strategic plan is designed to address the most pressing health needsidentified in Niagara County, aiming to improve the health and well-being of all residents and reduce health disparities.
