Golisano Center for Community Health
Located in downtown Niagara Falls, the Golisano Center offers primary care services in a setting designed to connect patients with the healthcare and community resources they need. In addition to primary care, the center provides access to behavioral health services, care management, wellness education, and other support programs.
Summit Healthplex
Located on Williams Road, Summit Healthplex provides convenient access to primary care in a modern outpatient setting. Patients also benefit from access to specialty services, imaging, rehabilitation, and other healthcare resources available throughout the Healthplex campus.

What is a Patient-Centered Medical Home?
Niagara Falls Memorial Medical Center has been a certified Patient-Centered Medical Home (PCMH) since 2021. Our certification as a PCMH by the National Committee for Quality Assurance (NCQA) means our patients can count on high-quality health care delivered with an emphasis on care coordination and communication. Comprehensive Care
NFMMC Primary Care Centers can meet most of our patients’ health care needs. Complete care takes a team effort. That team includes physicians, advanced practice nurses, physician assistants, nurses, clinical pharmacist, nutritionists, and care coordinators. Our Care Teams’ goal is to provide each patient with the best care uniquely suited to his or her specific needs.
Patient-Centered Care
