Grassroots Community Outreach is Niagara Falls Memorial Medical Center's neighborhood-based outreach initiative that connects underserved residents with healthcare, behavioral health services, transportation assistance, and community resources. By meeting people where they live, Grassroots helps remove barriers to care and improve the overall health and well-being of our community.
Grassroots Community Outreach
At Niagara Falls Memorial Medical Center, we believe healthcare begins long before someone walks through our doors. Through our Grassroots Community Outreach program, we take healthcare directly into Niagara Falls neighborhoods, connecting residents with the services, resources, and support they need to live healthier lives.
Led by our Population Health team and community volunteers, Grassroots focuses on reaching underserved residents where they live. By going door-to-door in targeted neighborhoods, our team helps connect individuals and families to primary care providers, preventive services, transportation assistance, behavioral health resources, and community support programs.
Meeting People Where They Are
Many residents face challenges that make accessing healthcare difficult. Grassroots helps remove those barriers by offering direct connections to care and scheduling appointments on the spot whenever possible. Individuals without reliable transportation can also be connected to transportation resources that help them reach medical appointments and essential services.
The Niagara Falls Memorial Mobile Health Van often accompanies outreach teams, providing convenient access to basic healthcare services including:
- Blood pressure screenings
- Glucose testing
- Wound care
- Sick visits
- Health education
- Preventive health services
Addressing More Than Medical Needs
Health is influenced by much more than medical care alone. Grassroots incorporates screening for Social Determinants of Health (SDOH) — factors such as housing stability, food access, literacy, and health insurance coverage that can significantly impact a person's overall well-being.
When needs are identified, Memorial connects residents with local organizations and agencies that can provide assistance. Through a growing network of community partners, the program helps ensure residents receive the support they need and that referrals lead to meaningful outcomes.
Expanding Access to Behavioral Health
Recognizing the importance of mental health, Grassroots also includes behavioral health outreach, helping residents connect with counseling, support services, and other mental health resources available throughout the community.
